Voluntary Surrender 

Voluntary Surrender of Certification, License, or Registration

  • A regulated person may voluntarily surrender the person's certification, registration, or license. The surrender must be submitted to the Director on the form approved by the Commission, and the form must be completed fully. The Director may request additional information.
  • The person must provide written notice of the voluntary surrender as required by the Director.
  • The circumstances under which the certification, registration, or license is surrendered will be noted in the person's records with the Commission and will be taken into account if the person applies to reinstate in the future.
  • A voluntary surrender does not affect any disciplinary matters before the Commission at the time of surrender.
  • The voluntary surrender is effective after the Director reviews documents submitted and forwards to Licensing section for processing. You will receive written notice of the surrender effective date when your certification is no longer active, and you will no longer be able to advertise or practice the services you were certified, licensed, or registered to practice.

To voluntarily surrender your certification, license, or registration, you must do the following:

  1. Submit a written request to: compliancedepartment@txcourts.gov. Ensure to reference your complaint currently before the Commission, including the Cause No. (if applicable).
  2. After your request is received, we will send you the Surrender License Form.
  3. Complete the Surrender License Form and return it with the following items:
    • a. your certification card, and
    • b. your wall certificate (if applicable).
  4. Mail the form and all required documents to the following address:
