Guardianship Annual Reporting
Please be advised that the Annual Reports you must submit for calendar year 2024 must include all of 2024 (i.e., January 1, 2024 through December 31, 2024); therefore, we are not accepting reports submitted prior to January 01, 2025. If you have already submitted a report, you will be required to resubmit it after December 31, 2024.
Texas Government Code Chapter 155.105, Texas Estates Code Sections 1104.302-306 and 1104.257-258 [formerly Probate Code Sections 697 and 697A], and Rule 7.3 of the Rules Governing Guardianship Certification impose requirements for county clerks, the Health & Human Services Commission Office of Guardianship (formerly known as DADS Guardianship Program), guardianship programs and private professional guardians to report certain information to the Judicial Branch Certification Commission. In addition, Estates Code Sections 1104.302-306 and 1104.257-258 [formerly Probate Code Sections 697 and 697A] require guardianship programs and private professional guardians to provide certain information to county clerks.
Reporting forms for calendar year 2024 will not be available prior to December 31, 2024. Reports for 2024 are due by January 31, 2025. Information must be reported for the entire calendar year, not as a "snapshot" as of the reporting date.

Note: Private professional guardians must submit original and signed reporting forms to the Commission by mail. All others may submit reporting forms by mail as shown on the forms. E-mail submissions are preferred.
